چکیده

Although early surgical repair for tetralogy of Fallot (rTOF) has significantly improved clinical outcomes and long-term survival, progressive right ventricle (RV) dysfunction driving morbidity and mortality is often observed in adults with rTOF. Management of RV heart failure is occasionally refractory to even appropriate medical therapies. RV mechanical dyssynchrony that is commonly observed in rTOF patients with right bundle branch block (RBBB) promotes depressed contractility and remodeling, and it is proposed as a desirable therapeutic target. We present 2 cases of rTOF complicated by severe RV heart failure, which were effectively managed with standard cardiac resynchronization therapy (CRT) with a second RV lead (CRT-RV).

Right ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease

Introduction:  In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...

بررسی عملکرد بطن‌ها با اکوکاردیوگرافی در نارسایی دریچه ریوی پس از جراحی تترالوژی فالوت

Background: Tetralogy of Fallot is the most common cyanotic congenital heart disease. The systolic and diastolic function in both ventricles is altered even after successful corrective surgery for this defect with a transannular patch. Pulmonary regurgitation, a common complication after this treatment, is usually well tolerated in childhood. The aim of this study was to assess the combined dia...

Outcomes of Pulmonary Valve Replacement for Correction Pulmonary Insufficiency after Primary Repair of Tetralogy of Fallot (TOF)

Background Total correction of Tetralogy of Fallot (TOF) anomaly in early childhood has been practiced in many centers with good results, but in some of patients after few years sever Pulmonary valve insufficiency occurred. Materials and Methods At a cross- sectional study from January 2015 to January 2016, 10 patients who had history of primary repair of TOF with free pulmonary insufficiency (...

Cardiac resynchronization therapy for management of congestive heart failure after repair of tetralogy of Fallot in an elderly patient.

A 65-year-old woman underwent total correction of tetralogy of Fallot with closure of ventricular septal defect, right ventricular outflow patch, pulmonary valve replacement and tricuspid valve repair. Pacemaker implantation using epicardial electrodes was simultaneously needed because she had complete atrioventricular block. The postoperative course was excellent. Eighteen months postoperative...

Assessment of right ventricular function by treadmill exercise stress cardiovascular magnetic resonance in patients with repaired tetralogy of Fallot

Background Cardiac MR (CMR) is considered the gold standard to assess right ventricular (RV) function in different scenarios, including cases of congenital heart disease. Moreover, an MR-compatible treadmill coupled with Real Time (RT) CMR has been developed for stress testing, and may be an optimal choice in this setting. The aims of this study were to determine the feasibility and accuracy of...
